Training class application
To register for a class, we need a
completed and signed application, plus a copy of your shot records.
All class dogs must be current on
rabies,
which is required by state law, and bordetella (kennel cough).
Additionally, dogs under 2 years should be current on parvo,
distemper, and Leptospirosis. Check with your vets to see what they
recommend. You do not need to pay until orientation night, although
it might save you a bit of a wait in line if you do. We strongly
recommend that you attend the orientation, which will cover learning
theory, training mechanics, safety, equipment, and house rules.
Please do not bring your dog to orientation.
- Please be sure to bring a copy of
your most recent shot records or ask your vet to fax them to us at
866-370-7821. Puppies need to have their second series before being
exposed to other dogs, all other dogs should be current on
vaccinations according to their vet's recommendation. Titers are
accepted.
- For payment, we accept cash, check
(please make payable to PDX), or charge (Visa or MasterCard only).

use good old USPS by mailing it to Play Dog Excellent, 4113 Dayton
Blvd, Chattanooga TN 37415. We will confirm receipt of all
applications via phone or email within one business day of receipt.
